Items needed to be uploaded for enrollment are:

• Birth certificate

• Immunization Records or Exemption Certificate

• Copy of IEP

• Prior Academic Records (if applicable)

• Medical Evaluation reports (Psychological, Neurological, etc.)

​

There will also be a $100 enrollment fee collected at the end of the form.
